Sample Disclosure Notice for the Student Application Process

Disclosure of your Social Security Number (SSN) is requested for the student records system of the University at Buffalo and for compliance with Federal and State reporting requirements. Federal law requires that you provide your SSN if you are applying for financial aid. Although an SSN is not required for admission to the University, failure to provide your SSN may result in delays in processing your application or in the University's inability to match your application with transcripts, test scores, and other materials. Supplying an SSN ensures that you will be able to claim federal tuition tax credits (such as the Hope Tax Credit), if you are eligible, on your federal tax return. The University has a strong commitment to ensuring the privacy and confidentiality of students records and will not disclose your SSN without your consent for any purpose except as allowed by law.

Sample Disclosure Notice for the Employment Process and General HR Usage

Federal tax regulations require us to request an SSN from every employee to whom compensation is paid. Disclosure of your SSN at the time that you apply for employment at UB is voluntary, but disclosure of your SSN is mandatory before you may be employed by the University. Employee SSNs are maintained and used by the University for payroll and benefits purposes, to verify employment history, and are reported to Federal and State agencies on forms required by law or for benefits purposes. The University will not disclose an employee's SSN to anyone outside the University without the consent of the employee except as allowed by law.

Sample Disclosure Notice for Payments to Individuals for Stipends, Honoraria, or Other Payments

You must provide a Social Security Number (SSN) in order for the University at Buffalo to process a __[state the type of payment]__. The University is required by Federal law to report such payments along with Social Security Numbers to Federal and State agencies on forms required by law. The University will not disclose a recipient's SSN withough the consent of the recipient to anyone outside the University except as mandated by law.

Sample General Mandatory Disclosure Notice

Disclosure of your Social Security Number (SSN) is requred of you in order for the University at Buffalo to __[State intended use of SSN]__, as mandated by the __[Federal][State]__ Law. The University will not disclose a recipient's SSN withough the consent of the recipient to anyone outside the University except as mandated by law.

Sample Voluntary Disclosure Notice

Disclosure of your Social Security Number (SSN) is requested in order for the University at Buffalo to __[state intended use of SSN]__. No statute or other authority requires you to disclose your SSN for that purpose. Failure to provide your SSN, however, may result in __[state what may happen if the individual fails to provide SSN]__. The University will not disclose a recipient's SSN withough the consent of the recipient to anyone outside the University except as mandated by law.
